Applying for FQHC Funding
If you are interested in establishing a new FQHC or adding a new access point for an existing FQHC, in order to qualify for funding, applicants must:
- Meet the legislative guidelines, the Program Expectations, and the governance requirements for FQHCs.
- Be located in an area designated as a Medically Underserved Area or Population (MUA/P). This designation should be in place or in process before applying for FQHC funding for New FQHC Access Points which are accepted two times a year, typically in December and May.
- Submit a Needs for Assistance worksheet (see Attachment C, FY 2005 New Access Point Grant Guidance)
- Submit a Letter of Interest describing the proposal project.
- Note* A minimum need for assistance score of 70 is required for consideration for federal funding.
