Uniform Data System (UDS)

Data Collected
A core set of information appropriate for monitoring and evaluating health center performance and reporting on trends is collected through UDS.

Grantees Required to Report
The UDS is a reporting requirement for grantees of the following primary care programs administered by the BPHC, HRSA:

  • Community Health Center
  • Migrant Health Center
  • Health Care for the Homeless
  • Public Housing Primary Care
  • and any section 330 funded Health Center

Use of Data
The data are analyzed to ensure compliance with legislative mandates, report program accomplishments and justify budget requests to the U.S. Congress. The data help to identify trends over time, enabling HRSA to establish or expand targeted programs and identify effective services and interventions to improve access to primary health care for vulnerable populations.UDS data are compared with national data to look at differences between the U.S. population at large and those individuals and families who rely on the health care safety net for primary care. UDS data also inform BPHC grantees, partners, advocacy groups and communities about BPHC programs and their users. 


What information is included in the UDS?

The UDS provides basic information about:

  • Number and socio-demographic characteristics of people served
  • Types and quantities of services provided
  • Types of staff who provide these services
  • Clinical characteristics of patients
  • Cost and efficiency of delivering services
  • Sources and amounts of income
NOTE:  The UDS for each individual center is not routinely released to third parties or posted in any public data location.  There is a state/regional rollup that is available for public consumption (UDS State/Regional Rollup Data).

Deadline for 2009 UDS submission is before or on March 31, 2010. No other changes will be permitted after this date.  (This is the language used by HRSA as of Sept. 29, 2009.)
