LPCA’s Quality Improvement Program
The quality improvement program is designed to serve as a resource to the members of LPCA by providing technical assistance in various areas. A charge for the quality improvement coordinator is to assist members in providing continued quality care to the more than 100,000 Louisiana recipients that are served on an annual basis. The coordinator's role is to develop, implement, and provide quality improvement initiatives that will provide training and technical assistance to our 18 grantees. In receiving the assistance offered by the quality improvement coordinator through health center surveys, critical need analysis, assisting in establishing priorities for training and technical assistance and aiding with the implementation of proper protocols for such priorities, our health centers are assured that they are providing the highest quality of health care to their patients. Developing and implementing programs and activities that will assure that our health centers provide high quality patient care and engage in appropriate monitoring, utilization review and clinical outcomes reporting are just a few of the benefits of the program. There is also a focus on practice management efforts that will include, but not be limited to, areas of:
Quality Improvement

Assisting with the compilation and aggregating of data for Performance Improvement Initiatives

Asses, develop and implement customer service programs that will help in identifying deficiencies according to present situations

Create action plans based on patient satisfaction survey scores
Performance Improvement

Conduct work flow analysis, time and motion studies to collect quantitative and qualitative work data

Review current or develop and coordinate data collection methodologies to enhance and sustain improvement opportunities

Perform detailed data analysis including trend identification and root cause analysis, then recommending changes in organization wide processes, policies, and procedures based on findings
Risk Management

Investigate and document the facts of incidents and communicate risk potentials to executive directors

Prepare statistical reports of individual, aggregate and departmental data which measures the type and scope of incidents

Present educational programs on risk management to physicians and staff in order to aid in identifying instances which jeopardize patient and employee safety
Corporate Compliance

Coordinate compliance audits to insure ethical behavior is maintained

Analyze and expand, if needed, an ongoing open line of communication at CHC’s

Investigate possible violations and report any with corrective action plan to Executive Director’s of CHC’s
The quality improvement program is charged with providing quality driven workshops, seminars, and trainings in the areas of billing and coding, evaluation management, quality management, and uniform data systems. The coordinator often teams with the National Association of Community Health Centers to bring renowned guest facilitators for these events. The vision of the quality improvement program is to support a continually evolving health care system that will result in optimized treatment to Community Health Center patients’ of Louisiana.
